CAP/DA and CAP/C are Medicaid programs that help qualified individuals receive supportive care and services in their own homes as an alternative to nursing home placement.

- Case Management provided by Registered Nurses and Case Managers
- In-home Aide Service
- Durable Medical equipment
- Medical Supplies, Incontinent Supplies, Nutritional Supplements
- Telephone/Lifeline Alert
- Home Modifications
CAP/Choice
This CAP option is consumer-driven care. Once approved for CAP/Choice, this program allows older and disabled individuals to have the opportunity to maintain increased control over their care, support, and services arranged to meet their needs.
Eligibility Requirements
- Cherokee County or Clay County residents
- Those who are eligible for full Medicaid
- Those at risk for nursing home placement
- Individuals who can be approved for skilled or intermediate care by a physician
- Individuals who can be cared for safely in their home at less cost than facility placement. This service is paid for by Medicaid.
