Community Alternative Program

CAP/DA and CAP/C are Medicaid programs that help qualified individuals receive supportive care and services in their own homes as an alternative to nursing home placement.

Some Available Support Services Include:
  • Case Management provided by Registered Nurses and Case Managers
  • In-home Aide Service
  • Durable Medical equipment
  • Medical Supplies, Incontinent Supplies, Nutritional Supplements
  • Telephone/Lifeline Alert
  • Home Modifications
CAP/Choice

This CAP option is consumer-driven care. Once approved for CAP/Choice, this program allows older and disabled individuals to have the opportunity to maintain increased control over their care, support, and services arranged to meet their needs.

Eligibility Requirements
  • Cherokee County or Clay County residents
  • Those who are eligible for full Medicaid
  • Those at risk for nursing home placement
  • Individuals who can be approved for skilled or intermediate care by a physician
  • Individuals who can be cared for safely in their home at less cost than facility placement. This service is paid for by Medicaid.
